How to report a concern

You can use your institution’s online forms to raise concerns about your own safety or wellbeing, or about another learner. You can also use this process to share general information.

If you raise a concern about another learner, they won’t see what you wrote and they won’t be told that a concern has been submitted.

There are two ways to submit information. The steps are the same, but each option is used for different types of reports.

Options for raising concerns

When raising a concern you can use the following options:

  • Add record - Use this option for serious concerns that require detailed information, such as welfare or mental‑health issues.

  • Add note - Use this option to share quick, simple information, such as details about medication.

Raise a concern

Report a concern as follows:

  1. Choose what you want to submit. Select Add Record or Add Note, depending on the type of information you want to share.

  2. Identify who the concern is about. Select the learner or indicate that it relates to yourself.

  3. Choose the form to complete. Your institution provides several forms so you can supply the information they need. Select the form that best fits your concern.

  4. Complete the form, required fields are marked with a red asterisk.

    You may be asked different types of questions:

    • Notification options - If you’re asked who should be notified, select any relevant contacts. If you don’t select anyone, the safeguarding team will still be informed.

    • Text boxes - Select the text box and type your response. Formatting options appear above the box.

    • Multiple choice questions - Answer by selecting from drop-down lists, check-boxes, or radio buttons.

    • Body map - Select the relevant location on the body map to add a red cross. Add as many markers as needed. To remove selections, choose Clear markers at the bottom of the map.

  5. Submit the form. Select Save as draft if you want to finish your submission later or select Submit to send your message.
